EJ Intake Forms
We request that you complete your intake forms prior to your appointment. These forms are for new guests and guests who have not been with us in over a year:
These policies allow us to provide the highest-level services and adhere to our values and mission while respecting our team members and all our guests.
- Reserving your appointment: A credit card will be required to make any reservation. We understand that placing a credit card on file may be an inconvenience however this helps to protect our service providers’ time as well as keeping EJ a thriving business and community resource. Due to an increasing incidence of missed appointments and last minute cancellations we feel it necessary to make this requirement.
- Appointment booked: Shortly after making your reservation, you will receive a text message and/or email with the booking information.
- Confirming your appointment:You will receive a text and/or email 48 hours prior to your reserved time. Please use the appropriate links to confirm or request to reschedule.
- Arrival Times: Please arrive on time for your appointment. For massages and body treatments, please arrive 15 minutes early. If you are more than 15 minutes late for any appointment, we reserve the right to cancel the appointment and reschedule to a later date.
- Cancellations: We require a firm 24 hour notice for cancellations. Cancellations with less than 24 hours notice are subject to a service fee in the amount of half the service price.
- Missed appointments and No Show: If an appointment is missed or you do not show for the appointment a service charge of half the service price will be incurred.
- Service satisfaction: Every effort is made to ensure your happiness with our services. Please notify us within 7 days if you are not satisfied. If any service corrections are requested later than 7 days from the original service, additional payment may be required. No refunds are issued for services received.
- Product satisfaction: We believe in the quality of the products we offer at EJ. If you are unhappy with a product please return within 14 days of original purchase. Purchase history or receipt required for full refund. After 14 days a credit toward another product will be issued
- Cell phone etiquette: Please considerate of others in the salon area. In the spa area please silence or place on vibrate to ensure the relaxation of all our guests. We request phone calls be taken outside.
- Gratuity: Suggested gratuity is between 15% and 20% of service cost. Gratuties can be received via Venmo, cash, and check. For credit card gratuity we utilize Tippy. The following fee applies to each Tippy transaction: $0.50 plus 2.75% of the transaction amount.
- Prices subject to change: While we make every effort to keep all information current, prices may change without prior notification.
